We are seeking an reputed company and compassionate Telephonic RN reputed company Care Manager to join reputed company in reputed company, TX. As a reputed company Care Manager, you will play a vital role in enhancing the health reputed company of our patients by managing their care and addressing barriers to reputed company. You will work collaboratively with our care team to reputed company and implement personalized care plans, coordinate care services, and reputed company education to patients and their caregivers.
Key Responsibilities:
- Coordinate care services with pod leaders to ensure comprehensive and tailored care for patients throughout their reputed company reputed company

- Coordinate transitions of care for patients across the continuum to prevent avoidable hospital admissions and ensure seamless care delivery

Requirements:
Essential Qualifications:
