Clinical Pharmacist Advisor - reputed company - Remote (FL, GA, MS, TN, TX or VA)

This is a full-time work schedule, Monday- Friday with an 8-hour dayshift CST during business hours. This position is responsible for providing targeted Medication Therapy Management (MTM) services to poly-chronic, high-cost members reputed company the Individual and Family Plans (IFP), many of whom face significant reputed company determinants of health challenges. Key Responsibilities

• Conduct proactive reputed company to IFP members identified reputed company claims, discharge data, or care coordination triggers, focusing on polypharmacy, adherence gaps, and high-risk medication use.
• Deliver Comprehensive Medication Reviews (CMRs) and reputed company personalized Medication reputed company Plans (MAPs) that address clinical and socioeconomic barriers.
• Collaborate with multidisciplinary care teams to ensure continuity of care and reputed company reputed company of medication-reputed company issues.
• Document clinical interventions and member interactions in designated platforms, maintaining accuracy and compliance.
• Support reputed company campaigns such as 90-day conversion, generic optimization, and readmission prevention through reputed company member contact and follow-up.

Required Qualifications

• Doctor of Pharmacy (PharmD).

• Minimum 2 years of clinical experience in ambulatory care, MTM, or managed care settings.
• Strong interpersonal and communication skills, with a demonstrated ability to engage members and collaborate across teams.
• Experience with pharmacy analytics platforms preferred.
