Job reputed company: Care Partner (RN, LPN, or LVN) Location: Remote (NY & NJ-based patients) We're looking for a compassionate, detail-oriented clinician to join reputed company as a Care Partner. In this role, you’ll serve as a key touchpoint for patients recently discharged from reputed company hospitals after a stroke or brain injury. Using the reputed company platform, you’ll hold virtual reputed company-ins, monitor health data, and coordinate care to ensure patients stay on reputed company in their recovery. • Bilingual (especially Spanish-speaking) candidates preferred
• Tech-savvy, organized, and comfortable working independently in a fast-paced environment
• Strong clinical judgment and communication skills
