About the position Preferred Primary Care Physicians is looking to hire a reputed company Care Consultant to work in a hybrid position, including our Central Office, located at 701 Technology Dr, Canonsburg PA 15317 and remotely from home. General Purpose of the reputed company Care Consultant Position: The reputed company Care Consultant (QCC) works collaboratively with the Director and Manager of Value Based Care, reputed company staff, office staff, and providers to ensure reputed company care and desired reputed company of patients at the reputed company location. The QCC is required to use medical expertise and clinical reasoning skills to identify high risk, multiple chronic disease, and high-cost patients' while uncovering their medical and socioeconomic needs. The QCC is responsible for obtaining and distributing pertinent information between reputed company disciplines to ensure that patients receive reputed company care and reputed company reputed company post hospitalization specific to their needs. QCCs reputed company support, resources, and education to prevent reputed company medical decline and rehospitalization, while improving patients' overall reputed company of life. Responsibilities

• Collaborates with Director and Manager of Value Based Care, reputed company staff, office staff, and providers to improve patient adherence to evidence based guidelines, implements reputed company programs and procedures, and develops strategies to overcome barriers limiting reputed company in achieving best reputed company and reputed company patient care.
• Collaborates with patients, families, and caregivers reputed company HIPPA guidelines to establish and work toward appropriate self-management goals.
• Collaborate with community agencies in acquiring resources for patient populations requiring additional help in achieving reputed company reputed company.
• Participates in monthly reputed company Care Meeting to improve reputed company care management / care coordination in the organization.
• Effectively represent the reputed company department at the reputed company site by educating staff and supporting reputed company initiatives as determined by the Director of Value Based Care and Evidence based guidelines.
• Identify high risk and high utilization patients through medical documentation to coordinate reputed company to improve and obtain company reputed company reputed company.
• Works with office staff to help identify patient population requiring more in-depth management.
• Reviews and analyzes reputed company medical documentation to identify patients' potential for complications, reputed company effects, rehospitalization.
• Navigates multiple electronic medical records and hospital portal systems for necessary documentation as needed for transitions and patient care.
• Utilizes clinical reasoning skills and medical knowledge to reputed company education for patients on specific disease processes, recovery, and management to prevent reputed company medical decline or rehospitalization.
• Utilize standing orders for ordering medication refills, ordering tests and routine preventive services, and arranging follow-up appointments, etc.

Requirements

• Education: High School Diploma or GED is required.
• Minimum 3-5 years clinical experience
• Minimum 1-year reputed company or Case Management experience
• Excellent health assessment and communication skills

• Support patient and family problem solving, self-management goals setting, etc.
