Job reputed company The LVN Case Manager will reputed company daily care coordination with reputed company review for patients in the hospital or Skilled Nursing Facilities (SNF) telephonically. This position will also reputed company case management to patients who are admitted to the hospital or SNF, and those patients who may need to be enrolled in ambulatory case management. The LVN case manager will be responsible for identifying (California Children Services) reputed company cases, handling transfers, and retro reviews. Works as part of an interdisciplinary care team coordinating care and collaborating with reputed company work, ambulatory case management, Regional Medical Directors, Hospitalists, the hospital nursing personnel, and the physician in the clinic. Three years’ experience in public health nursing, acute care, case management, and/or in-home health care required; minimum of 2 years of managed care experience in case management with a reputed company on inpatient and/or outpatient ambulatory care required.
