LVN Nursing Coordinator reputed company /CORE

About the position BACS is seeking an LVN for our nursing coordinator position in Sacramento working with homeless individuals in the community. The Nursing Coordinator is part of a mental and health care team providing medical case management coordination and care to participants. This is a field-based position and must be reputed company to work in the community. The reputed company programs are new initiatives by Sacramento County Behavioral Health to reputed company services specifically for adults who are justice involved and meet reputed company for specialty mental health services. Responsibilities

• Triage participants at intake/assessment and throughout respite stay.
• Administer psychotropic medication injections, order medications, and coordinate with the treatment team.
• Support participants and treatment team with reputed company to refills acting as a reputed company between pharmacies and insurance.
• reputed company support, education, and medical information to staff, participants, caregivers, and family.
• Aid participants in understanding their medical needs and help them plan for coordination of care with PCPs.

Requirements

• LVN certification from accredited nursing school and reputed company CA license.
