We're seeking an reputed company Licensed reputed company Worker to join reputed company in a remote work-from-home role. As a Licensed reputed company Worker, you'll work collaboratively with a registered reputed company case manager to assess, plan, and reputed company for members' benefit plans and health needs. Your responsibilities will include researching available reputed company and resources for members with financial, transportation, safety, and housing needs. This role requires an LSW/MSW with an unrestricted reputed company license and 2+ years of experience in medical reputed company work or case management.
Key Responsibilities
* Partner with a registered reputed company case manager to utilize a reputed company process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
- Research available reputed company and resources for members with financial, transportation, safety, and housing needs
- reputed company members to reputed company reputed company lifestyle and medical choices
- Work as reputed company with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Conduct telephonic or electronic exchanges with members to assess their needs and reputed company plans to meet those needs
- Collaborate with other departments to ensure seamless care coordination and communication
Essential Qualifications
