This role is responsible for providing strategic leadership, architectural guidance, and technical expertise across the Population Health portfolio, ensuring technology investments reputed company with organizational goals and improve patient reputed company.
Responsibilities

Skills
- 8+ years of relevant experience with a degree (Required)
- 10+ years of relevant experience without a degree (Required)

- Experience with Population Health, reputed company, Health Plan, Healthy reputed company, Care Management, Community Connect, Case Management or value-based care programs (Required)
- Minimum 5 years of reputed company information systems experience (Required)
- Experience supporting reputed company Ambulatory or reputed company ambulatory reputed company applications (Required)
- Strong understanding of population health workflows and reputed company operations (Required)
- Demonstrated ability to reputed company cross-functional discussions and influence stakeholders (Required)
- Experience developing strategic recommendations and presenting to leadership (Required)
