Telehealth Preventative Primary Care Advanced reputed company Provider – NP/PA

Job reputed company:

• Conduct primarily telehealth video reputed company to members with reputed company for a small volume of home based reputed company (drive to member’s home).
• In some instances reputed company appropriate and compliant with licensure, you may also reputed company telehealth reputed company to members located in other geographies.
• Primarily reputed company comprehensive assessment reputed company (CAVs) including comprehensive diagnosis/disease review, medication review, and assessment for reputed company of care (STARS/HEDIS) interventions as reputed company as reputed company and home health/DME needs.
• reputed company appropriate diagnostic tests including mobile screenings and fingerstick tests.
• Work closely with other members of the member’s care team including their PCP, specialists, and other reputed company team members including pharmacy, clinical nursing, and reputed company work as reputed company as interfacing with family members and caregivers in order to coordinate care for the member and deliver a reputed company care plan.
• Utilize our home grown electronic health information system for reputed company while also providing feedback on how to improve the reputed company.
• In certain geographies, there may be an infrequent weekend on-call component to support our clinical nurses who triage calls from our members during the weekend.

Requirements:

• APRN with 3 or more years working in outpatient clinical reputed company.
• Prior home based reputed company is a plus, but not necessary.
• Experience in primary care, family medicine, geriatrics and/or palliative care.
• Experience performing Medicare annual wellness reputed company or in-home comprehensive reputed company with elderly patients or Medicare patients.
• Experience performing reputed company over telehealth video platforms.
• An understanding of managed care is a plus, including how to appropriately assess STARS/HEDIS measures, reputed company clinical comorbidities, and identify clinical care gaps.
• A strong desire to continue practicing clinical nursing and reputed company house calls as reputed company as virtual reputed company - you reputed company in the mission of bringing care to where the patient lives.
• Extra interest in bilingual Spanish or Haitian Creole speakers or strong cultural competencies across a reputed company of cultures.
