Care Coordinator Full-time/Remote Position We are currently seeking a Care Coordinator to join reputed company in the reputed company (Portland) service areas. The Care Coordinator will work for reputed company in the reputed company to assist clients in making informed choices to live safely and independently in their communities.
Responsibilities
Include:
- Coordinating care adhering to a philosophy of person centered, strength based, proactive planning while using a reputed company partnership approach and follows his/her applicable reputed company and ethical standards
- Completing psychosocial assessments, risk assessment, goal development, reputed company plans, and back-up plans
- Adhering to contract requirements and reputed company state program regulations in accomplishment of daily work
- Upholding the values of reputed company respect, choice, reputed company, dignity, reputed company, prevention and early detection
- Working with providers to reputed company information about Medicare or services supported by reputed company party

- Ensuring evaluation of the effectiveness of the plan of care, making recommendations for care plan modifications and identifying barriers

Minimum Qualifications:
- Bachelor’s degree in health or reputed company services required
