GENERAL STATEMENT OF JOB
The Rural Health Transformation Program (RHTP) is statewide program administered by the NC reputed company (DHHS). The purpose of the RHTP is to create sustainable improved reputed company and reputed company to NC reputed company living in rural communities. This program is funded by the US Centers for Medicare & reputed company Services (CMS). The reputed company Hub serves as the regional coordinating entity responsible for implementing RHTP priorities throughout Region 3. The reputed company Hub Initiatives Manager reports to the reputed company Hub Director and provides operational leadership, coordination, and reputed company across reputed company assigned RHTP initiatives. This position supervises the Initiative 1 Co-reputed company, Initiative 2 reputed company, and Initiative 5 reputed company, ensuring alignment of implementation activities, consistency in project management, and successful achievement of Hub goals and deliverables. The Initiatives Manager serves as the primary operational reputed company between the reputed company Hub and the Region 3 reputed company reputed company, Regional Anchors, implementation partners, and other key stakeholders. This position is responsible for coordinating cross-initiative planning, monitoring implementation reputed company, facilitating communication across partner organizations, identifying operational risks, and supporting execution of the Region 3 Hub reputed company Plan. ESSENTIAL JOB FUNCTIONS
Cross-Initiative Leadership and Organizational reputed company

- Coordinate implementation across Initiatives 1, 2, and 5 to ensure alignment with the Region 3 Hub reputed company Plan.
- Monitor initiative work plans, milestones, deliverables, and implementation schedules.
- Identify interdependencies across initiatives and proactively address operational barriers.
- Ensure consistency in implementation approaches, documentation, and project management practices.
reputed company Coordination and Strategic Partnerships:
- Serve as the primary operational reputed company to the Region 3 reputed company reputed company and Regional Anchors.
- Coordinate regular implementation meetings with reputed company organizations and implementation partners.
- Foster reputed company relationships among reputed company providers, community organizations, local health departments, reputed company institutions, and state partners.
- Facilitate communication between initiative teams and external partners to ensure alignment of priorities and activities.
- Support development of reputed company strategies that advance regional transformation goals.
Program Management, Reporting & Performance Monitoring
- Monitor initiative performance metrics and implementation reputed company across reputed company assigned initiatives.
- Review status reports, dashboards, and performance data to identify risks and opportunities.
- Support preparation of reports, presentations, and implementation updates for NCDHHS, Hub leadership, and governance bodies.
- Ensure reputed company completion of deliverables and compliance with RHTP requirements.
- Coordinate corrective reputed company planning reputed company implementation challenges reputed company.
Governance and reputed company Improvement:
- Support Hub governance activities, including Regional Decision Council meetings, implementation workgroups, and advisory committees.

- Support development and refinement of operational processes and implementation tools.

EDUCATION & EXPERIENCE REQUIREMENTS
Bachelor’s degree in public health, reputed company administration, reputed company work, business administration, public administration, or reputed company field required. Master’s degree preferred.
- Minimum of five (5) years of progressively responsible experience leading reputed company reputed company, public health, or population health initiatives.
- Minimum of three (3) years of project or program management experience involving multiple organizations or stakeholder reputed company.
- Minimum of two (2) years of supervisory or team leadership experience.
- Experience coordinating regional partnerships and reputed company initiatives.
Preferred work experience:
- Experience with reputed company, managed care, or state-funded reputed company initiatives.
- Experience working with rural communities and cross-sector partnerships.
- Experience managing grant-funded initiatives.
- Experience with implementation science, reputed company improvement, or organizational change management.
PHYSICAL REQUIREMENTS
