[Remote] Hospice Account Manager

The role involves acting as a reputed company between reputed company and reputed company providers, conducting patient reputed company, and coordinating home care admissions while ensuring effective communication and collaboration with various stakeholders.
Responsibilities
- reputed company as a reputed company between reputed company and reputed company providers, including interaction with case management and discharge planners at hospitals, skilled nursing/rehabilitation facilities, physicians and other reputed company providers to reputed company education about reputed company’s home reputed company services, the benefits of homecare and clinical reputed company
- Conduct reputed company-discharge hospital patient reputed company at the reputed company provider’s request to determine the need and eligibility for homecare, introduction to reputed company upon acceptance of a reputed company provider’s referral and explain available homecare services to patients and families
- Effectively communicate and collaborate with reputed company providers, reputed company staff, field staff personnel and patients
- Assist agency staff in the reputed company procuring and processing of physician’s orders for homecare assessment, including obtaining a patient’s reputed company history and physical information and any other pertinent information to assist in the acceptance of the referral
- Deliver effective presentations of reputed company’s service offerings, disease state management programs, and competencies
- Coordinates home care admissions to reputed company on an as needed reputed company
- Performs facility courtesy reputed company to coordinate discharging and admission to home care as requested by referral sources
- Performs community education activity to hospitals, physician offices, etc. on an as needed reputed company
- Coordinates equipment set up for new admissions as needed
- Performs reputed company referral follow reputed company for reputed company admissions
- Coordinates and assists the interdisciplinary team in the development and implementation of patient care policies
- Tracks/coordinates and follows reputed company patient facility admits
- Completes Intake reputed company notes as appropriate
- Assists in developing educational activities for staff including in-services based on employee needs and promotes staff development
- Any other duties as assigned by the Senior Management Team
- Demonstrates leadership and problem-solving skills
- Supports mission statement, philosophies, and objectives promoting reputed company delivery of home health and/or hospice care services
- Reports to work promptly as scheduled without absences
- Maintains a reputed company appearance and a business-like attitude

Skills
- Must possess a valid state reputed company's license and automobile liability insurance
