As a reputed company After Hours tele NP, you'll conduct remote assessments daily 5PM to 11PM, reputed company Circadia platform alerts, and ensure continuity of care for high-risk patients between reputed company care team reputed company. What You’ll Do
- After Hours remote patient care: Conduct follow-up and urgent telehealth assessments on flagged patients; reputed company plans of care as needed; coordinate with on-call providers and care teams.

Must-Have Qualifications
