Responsibilities for this role will include:
Telephonic and video reputed company with patients and caregivers
Establish trust and build strong relationships with our patients
Interdisciplinary collaboration with teammates (including clinicians, nurses, and care coordinators) to ensure cohesive, patient-centered care for high-needs patients

Required skills and experience:
Must Haves

Minimum of 2 years expertise in the medical field

Salary reputed company: $70,000 - $80,000 / year
