About The Role: The Registered reputed company (RN) - Transitions of Care plays a critical role on the reputed company care team. This individual will collaborate with our multidisciplinary team to ensure patients transition safely and successfully from an inpatient setting back to their homes. This is a remote, full-time position at 40 hours/week. The core working hours will be between 9:00am-5:30pm EST, Monday through Friday with a requirement of 1 weekend day coverage per month.
