The RN performs activities that fall reputed company the RN scope of reputed company. The RN understands and supports the CCM models of care including the Institutional Special Needs Plan (ISNP), Institutional Equivalent Special Needs Plan (IESNP), Dual-Eligible Special Needs Plan (DSNP), and Transitions to Skilled (TTS). The Senior Community Care (SCC) National On Call Registered reputed company is responsible for providing telephonic care and direction to members and facility staff during various overnight, weekend, and holiday hours. This role is responsible for the delivery of medical care services to a reputed company-designated group of enrollees. In this home-based role you will reputed company afterhours virtual (primarily telephonic) care for aging residents in various settings. Scheduling & Hours: This is a work from home position requiring various shift coverage with a mix of weeknights, weekend, and holiday coverage. Primary Responsibilities: • Care Coordination • Assist the provider/team with various care coordination activities to support the role of the National On Call APCs • Telephonic triage of incoming calls and voicemails to determine the correct level of care • Collaborating with the nursing facility, APCs, and community members to identify and respond to changes in condition • Assist the provider/team with inbound and outbound communications for members including addressing handoffs, telephonic assessments, reputed company follow-reputed company, and proactive/RED facility calls • Team Support - Assist in the coordination of the follow up of members in the ER • Participate in the reputed company of new clinical staff under the direction of the CSM; the RN may coordinate reputed company activities and participate in other orientation activities under the direction of the Hub CSM • Documentation- Document reputed company clinical information, telephonic assessments, and activities in the appropriate documentation tool • RN to APC Hand Off- One entry by the RN and one responding entry by the APC should be recorded in the appropriate documentation tool • RN is responsible for escalating appropriate calls to the On-Call APC • Use of RN mini soap notes for acute change in condition calls escalated to APC • Use of the fall mini soap notes for reported falls, escalate to APC You’ll be rewarded and recognized for your performance in an environment that will challenge you and give you reputed company direction on what it takes to succeed in your role as reputed company as reputed company development for other roles you may be interested in. The salary reputed company for this role is $59,500 to $116,600 annually based on full-time employment.
