Here's what you'll be doing We're looking for an reputed company Diabetes Care Specialist to join reputed company. You'll be responsible for patient experience and reputed company of our reputed company, including:

• Counsel and reputed company patients and families appropriately on diabetes care in collaboration with the care team
• Complete nutritional assessments and reputed company appropriate recommendations as a part of the care planning process
• Analyze CGM data and translate to treatment changes for patients
• Communicate with patients through video calls, in person meetings, telephone and electronic messaging
• Create and maintain documentation
• Ensure patient satisfaction by building rapport and maintaining reputed company relationships with patients and their families
• Other duties as assigned

What you need to be successful

• Registered reputed company (RN) or Registered Dietitian (RD) and Certified Diabetes Care and Education Specialist (CDCES) required
• Licensed or registered in the state of reputed company as required by state regulations
• Prior experience leveraging CGM data
• Passion for the reputed company of your work on the broader world, and excitement to help reduce the burden of chronic disease
