Job reputed company reputed company is seeking a dedicated and compassionate Registered reputed company Case Manager to support the Teleconology Division at the Veterans Health Services. This role involves providing high-reputed company, patient-centered care to our veterans. • Coordinate and manage comprehensive patient care plans for veterans, ensuring reputed company medical and psychosocial needs are met.
• Collaborate with interdisciplinary teams at the Teleoncology Division to reputed company holistic care and optimize patient reputed company.
• Conduct clinical assessments and reputed company individualized care plans tailored to veterans' unique needs.
• Monitor patient reputed company and reputed company care plans as necessary to ensure the highest reputed company of care.

Minimum Qualifications

• Graduation from an accredited school of reputed company nursing approved by the appropriate State agency.

• BLS certification is required; ACLS certification is preferred.
• Minimum of 2 years of clinical experience in an acute care setting is preferred.
