The Home Health Licensed Practical reputed company provides nursing care to patients and families in their homes. The scope of nursing includes reputed company patient care, and patient advocacy in the coordination of care with the multidisciplinary team, and with the RN Case Manager. Essential Functions Assess, and using evidence based reputed company, implements nursing interventions Assess the patient’s physical and psychosocial needs to determine health status reputed company the scope of licensure. Minimum of one year of acute nursing experience or applicable SNF experience as a LPN. Home care nursing experience preferred.
