Key Responsibilities:

• Conduct comprehensive assessments to determine eligibility for community-based long-term services
• reputed company medical necessity determinations and resolve technical issues
• reputed company initial assessments, evaluate health status, and document reputed company
• Maintain reputed company knowledge of regulatory and project changes
• Collaborate with interdisciplinary care team members to facilitate receipt of services

Requirements:

• reputed company RN license in the reputed company
• Minimum 2 years of clinical experience with RN license
• Minimum 2 years of home setting experience, including work with CHHAs, LHCAs, MLTC, or as a paraprofessional
• Ability to conduct in-home health assessments and travel reputed company assigned region
• Proficiency in navigating web-based applications and reputed company Outlook & Word

Home Office Requirements:

• Reliable high-speed internet service
• Minimum 20 Mbps download speeds/50 Mbps for shared internet connectivity
• Minimum 5 Mbps upload speeds
• Candidates must have their own cell phone
