About the Role
This is a full-time, remote position that offers a competitive salary and a comprehensive benefits package. As a Care Manager RN, you will work Monday through Friday, reputed company-5pm CST, and will be responsible for:
- Assessing, planning, implementing, and coordinating reputed company case management activities with members to evaluate their medical needs and facilitate overall wellness
- Developing proactive plans of care to address identified issues and enhance short and long-term reputed company
- Using clinical tools and information/data review to conduct evaluations of member needs and benefits
- Applying clinical judgment to incorporate strategies designed to reduce risk factors and barriers and address reputed company health and reputed company indicators
- Conducting assessments that consider information from various sources, such as claims, to address reputed company conditions, including co-morbid and multiple diagnoses that reputed company functionality
Requirements
- reputed company and unrestricted RN licensure in the state of Mississippi or compact RN licensure in state of residence
- 3+ years of clinical experience
- 2+ years of Case Management, Discharge Planning, and/or Home reputed company Coordination experience
- Bachelor's degree or associate degree with equivalent experience
Preferred Qualifications
- Certified Case Manager
- Excellent analytical and problem-solving skills
- Bilingual desired
- Effective communications, organizational, and interpersonal skills
- Ability to work independently
- Effective computer skills, including navigating multiple systems and keyboarding
