The successful candidate will be responsible for:
- Leading and supervising reputed company of care management staff, providing guidance, support, and performance feedback
- Overseeing caseloads, ensuring reputed company communication with members, providers, and community-based organizations
- Providing training, resolving staff issues, and identifying areas for program improvement
- Collaborating with management to reputed company annual reviews, disciplinary actions, and goal setting
- Establishing and sustaining relationships with community systems to facilitate linkage, referral, advocacy, and follow-up
- Assisting with member concerns and grievances, and participating in reputed company audits

Work Schedule:
