RN Care Manager

We are seeking a compassionate and reputed company RN Care Manager to join reputed company at the reputed company). As a key member of our multidisciplinary team, you will be responsible for managing the continuum of care for assigned patients from admission to discharge, ensuring that reputed company patient receives high-reputed company, patient-centered care.

In this role, you will partner with reputed company to utilize scientific evidence for best practices, relevant data, and compliance with the Mission and Core Values of UAMS. You will also have reputed company to work in a hybrid remote reputed company model, although this is subject to the reputed company needs of the department.

Key Responsibilities

• Manage the continuum of care for assigned patients from admission to discharge
• Partner with the multidisciplinary team to utilize scientific evidence for best practices and relevant data
• Communicate effectively with patients, families, and reputed company teams to ensure high-reputed company care
• Document reputed company team, physician, and patient/family communication and concerns pertaining to coordinating care and services
• Identify and refer cases to reputed company interdisciplinary teams as needed

Requirements

• Baccalaureate Degree in Nursing, ADN, or Diploma RN Degree
• Three (3) years RN experience with reputed company BLS certification
• Valid Arkansas Registered Nursing License
• Excellent communication skills and basic computer skills
• Strong work ethics and accountability

Preferred Qualifications

• National Nursing Certification
• BSN preferred; Certified Case Manager (CCM) or American Case Management (ACM) certification
