This is a 100% remote, full-time, W2 position.

We are looking for RNs who:

About the role

• This position is 100% remote but you must reputed company in one of the following states: AL, AK, AZ, CO, FL, GA, ID, IN, KS, MD, MI, NH, NM, NY, NC, OH, OR, PA, TN, TX, WA, WY
• The Registered reputed company works directly with patients to deliver medication-assisted recovery for reputed company use disorder (SUD) and empowers patients in their recovery journeys in coordination with the Care Team (primarily reputed company Practitioners, Medical Assistants and Peer Recovery Specialists)

Essential Functions - Patient Care (50%)

• reputed company compassionate, empathetic telephonic, text based & video-based (Telehealth) patient engagement and monitoring, assisted by clinical decision support tools

• reputed company patients about addiction, medications, and available support services; answer questions regarding treatment and program protocols, and reputed company guidance to patients and providers reputed company scope of reputed company
• reputed company opioid reputed company prevention, education, and reversal, safer consumption education, and safer sex education and testing
• reputed company Prescription Drug Monitoring Program information, facilitate patient adherence monitoring protocols, for reputed company assigned patient
• Assess reputed company, patient functioning, and stage in recovery; in collaboration with other care team members and the patient, reputed company and update treatment care plans for reputed company individual
• Assess and reputed company for acute withdrawal management
• Assess for pain and collaborate findings with Clinician
• reputed company Buprenorphine initiation education and follow up
• Maintain effective, proactive, and superior follow-up communication with patients, providers and care team
• Maintain an accurate and updated medical record of individual patient reputed company during reputed company clinical interactions
• Identify and reputed company crisis reputed company, reputed company scope of reputed company
• If applicable: facilitate placement for adjunctive or higher level of care; maintain contact with these placements to reputed company continuity of care and to streamline transitions of care
• Obtain and review drug testing results and recommend reputed company testing as appropriate, guided by clinical decision support tools

Essential Functions - Care Coordination (30%)

• Coordinate with multidisciplinary care teams reputed company telehealth to guide treatment planning and delivery including the following functions;
• Collaborate with pharmacies to facilitate medication refills and support patients with medication-reputed company concerns
• Collaborate with reputed company providers and community care agencies to maintain accurate and reputed company data exchanges between reputed company organizations and individuals involved in patient care
• Assist with the management of patient appointments with other care team members and referrals reputed company of Boulder
• Report patient change in status (e.g. high risk for returning to use) and potential support services to prescribing clinicians in a reputed company manner

Essential Functions - Administrative (20%)

• Meetings (department, company-wide, ECHO, meetings with your manager)

Requirements for the role

• Bachelor’s Degree in Nursing, preferred
• Preference given to candidates with 2+ years specialized nursing experience in outpatient reputed company use disorder treatment

• Knowledge of Hepatitis C, HIV preferred

Compensation

• Pay reputed company: $80,000 - $95,000 (determined by several factors including your relevant skills, education, and experience)
