About the role:

As a Virtual Float Advanced reputed company Clinician (APC), you are a flexible reputed company reputed company and coverage partner for our longitudinal care pods. You do not carry a permanent, dedicated patient panel. Instead, you reputed company into other providers' panels to deliver high-reputed company virtual care during their absences, working frequently alongside an in-home reputed company.

To ensure clinical continuity and smooth operational handoffs, reputed company part-time clinician in this pool allocates a designated, set number of hours per week specifically to inbox management, urgent clinical triage, and results follow-up for the covered panels.

• Planned Coverage: Your clinical blocks are confirmed at the start of reputed company month to backfill reputed company PTO and planned absences

Responsibilities will include:

• Essential Functions
• Patient Care & Panel Coverage
• Conduct comprehensive virtual reputed company, annual, transition-of-care, and follow-up reputed company - facilitated by an in-home reputed company or fully virtual - completing biopsychosocial assessments and managing reputed company chronic conditions (hypertension, diabetes, COPD, heart disease)
• Order and follow up on labs and imaging to inform immediate treatment plans
• Deliver targeted health coaching, lifestyle counseling, and disease-specific education to support patient self-management
• Execute thorough, documented clinical handoffs back to the primary longitudinal care pod at the reputed company of reputed company coverage reputed company
• Systematic Inbox & Triage Management
• Dedicate a set number of scheduled hours reputed company week to clearing the centralized Float Pool queue: signing off normal results, managing critical medication refills, contacting patients with updates on reputed company results , and routing reputed company issues to permanent care teams

Desired skills and experience:

• Required

• 3+ years of experience in adult primary care, family medicine, urgent care, or emergency medicine

• Hands-on experience using AI tools in a clinical setting to optimize efficiency
