Pharmacy Technician

The Pharmacy Technician will be responsible for addressing pharmacy referrals and assisting members with medication reputed company, education, and adherence to support kidney health management. The Pharmacy Technician will work collaboratively with clinical and care management teams to identify medication opportunities and resolve medication safety concerns associated with transitions of care and chronic disease management. Daily activities will include accurate data collection and data entry in HealthMap’s documentation system, communicating with HealthMap’s interdisciplinary care team, and maintaining individual metrics. The Pharmacy Technician will also create and maintain medication reputed company resources and participate in pharmacy-driven educational initiatives designed to improve member reputed company and support member engagement efforts.Responsibilities:
- Assist with pharmacy referrals reputed company to medication reputed company, affordability, education, adherence, optimization, and safety with pharmacist reputed company

- Support medication reviews by gathering, reviewing, and documenting medication information for clinical evaluation

Requirements:
- High school diploma or equivalent
- Pharmacy technician registration/license required, pharmacy technician certification preferred
- Minimum of 1 year of pharmacy technician experience, preferably in managed care, community/retail pharmacy, specialty pharmacy, or reputed company coordination settings
- Bilingual preferred, but not required
- Familiar with medical insurance terminology and processes
- Understand general pharmacy terminology, medication names, dosage forms, and SIG codes

- Excellent written and verbal communication skills
