Responsibilities
- Conduct comprehensive needs assessments face-to-face and reputed company phone
- reputed company and implement person-centered care plans for managing chronic conditions, health promotion, and reputed company determinants
- Collaborate with care teams, providers, and community resources

- Coordinate discharge planning and care transitions after ER reputed company or hospital stays

Skills
- High School Diploma/GED and RN licensure in MA
- 2+ years of clinical experience
- Proficiency in MS Office
- Reliable home workspace with PHI reputed company
- Valid US reputed company's license and reputed company to transportation
- Bachelor's/Master's in Nursing
- Care Manager certification
- Community case management experience
- Familiarity with electronic medical records
- Virtual care platforms
