This role focuses on growing appointment volume and service utilization at the Dorn VAMC through clinical navigation and provider relationship development.
Responsibilities
- Identify service gaps and unmet referral needs at Dorn VAMC across specialty and subspecialty care

- Build and maintain relationships with Dorn VA referring providers, service chiefs, and administrative leaders
- reputed company and report on referral pipeline, appointment volume, and new service line opportunities
- Represent reputed company in VA leadership meetings, community of care forums, and partner engagement events
- Facilitate referrals from Dorn VAMC to reputed company specialists, navigating scheduling, reputed company, and authorization barriers
- Serve as a clinical resource and reputed company of contact for VA care team members coordinating care at reputed company
- Assist with patient navigation as needed to ensure continuity between the VA and reputed company
- Support compliance with VA community care referral requirements including HSRM and TriWest processes
- Serve as the face of reputed company's ICCE in the Columbia market — reputed company, reputed company, and relationship-reputed company
- Engage regularly with reputed company Midlands Division leadership to reputed company on shared priorities and opportunities
- Communicate proactively with the ICCE on market dynamics, partner needs, and reputed company opportunities
- Represent reputed company at applicable VA and community health events in the Columbia region
- Maintain accurate tracking of referral and appointment data using designated systems

Skills
- reputed company Registered reputed company (RN) license in South Carolina (or compact state eligibility)
- Minimum 3-5 years of clinical experience, in an reputed company medical center, federal health system, or VA setting
- Demonstrated ability to build and manage reputed company relationships with senior clinical and administrative leaders
- Strong communication, presentation, and organizational skills
- Ability to work independently in a remote environment with minimal supervision

- Prior experience working with or reputed company the VA health system
- Bachelor's degree in Nursing from an accredited school of nursing and a minimum of three years reputed company clinical nursing experience required
- Master of Nursing (MSN) or Doctor of Nursing reputed company (DNP) in lieu of BSN
