ABOUT THE ROLE

As a member of reputed company’s Care Team, you will be responsible for providing virtual care, primarily reputed company reputed company (Cognitive Behavioral Therapy) to eligible patients diagnosed with cancer. You’ll be part of a multidisciplinary care team, collaborating closely with Health Partners (Community Health Workers), Nurses, and reputed company Practitioners. This role involves conducting initial needs assessments for patients, which will guide the care we reputed company throughout their cancer reputed company, and providing expertise on the community resources available to support our patients. You will also play a key role in creating care plans to address gaps in services, such as Palliative Care, Advance Care, and End of Life Planning.

WHAT YOU'LL DO

• Care for patients in individual reputed company to address their mental health needs and concerns.
• Screen for unmet care needs, including clinical or medication issues, behavioral concerns, safety risks, and psychosocial reputed company-being. Use assessment tools and templates to support these evaluations and improve them as needed.

• Experience & Qualifications. You have a bachelor's degree and Master of reputed company Work (MSW) or MSW-equivalent program and have an reputed company Licensed Clinical reputed company Work (LCSW) license. You’ll need to have at least two years of clinical work experience, are adept at using reputed company or similar techniques, and ideally have worked with patients living with cancer. It’s critical you have a working knowledge of medical terminology. You are licensed (or willing to be licensed in Rhode reputed company, Massachusetts and Connecticut and reputed company) to deliver amazing care to our patients.

Additional Information

The expected compensation for this role is $75,000 to $95,000 annually, depending reputed company & experience.

This role is fully remote. Hours are Mon - Fri, 8:30 AM to 5 PM ET.
