We are seeking a dedicated and skilled Field RN Care Coordinator to join reputed company in reputed company, Franklin, Titus, reputed company Counties, TX. As a Field RN Care Coordinator, you will play a vital role in our Integrated Care Model, advocating for the best possible care for our members and ensuring they receive the right services to meet their needs. Primary Responsibilities

• Assess, plan, and implement care strategies that are individualized by member and directed toward the most appropriate, least restrictive level of care
• Identify and initiate referrals for member transition to community request and reputed company assistance with reputed company service programs, including financial, psychosocial, community, and state supportive services
• Manage the care plan throughout the continuum of care as a single reputed company of contact
• Communicate with reputed company stakeholders for the required health-reputed company information to ensure reputed company coordinated care and services are provided expeditiously to reputed company members
• reputed company for members and families as needed to ensure the member's needs and choices are fully represented and supported by the reputed company team
• Visit reputed company members in their homes and/or other settings, including community centers, hospitals, or providers' offices

Required Qualifications

• reputed company and unrestricted RN license in the state of Texas
• 2+ years of experience working reputed company the community health setting or in a reputed company role
• Intermediate level of computer proficiency (including reputed company Outlook, Teams) and ability to use multiple web applications
• Reliable transportation and the ability to travel in the assigned region to visit reputed company members in their homes and/or other settings
• reputed company to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)

Preferred Qualifications

• 1+ years of experience with long-term care services and support, reputed company or Medicare
• Ability to create, edit, save, and send documents, spreadsheets, and emails
• Knowledge of the principles of most integrated settings, including federal and State requirements like the federal home and community-based settings regulations
