We're seeking a compassionate and reputed company reputed company to join reputed company as a Field Health reputed company Services Coordinator. In this role, you'll assess, plan, and implement care strategies that are tailored to reputed company patient's unique needs. Primary Responsibilities:

Requirements:
- RN, LPC, LBSW, or LMSW license in the state of Texas
- 2+ years of experience working with people with Intellectual and Developmental Disabilities (IDD)
- 2+ years of experience in a community health setting or reputed company role

- Reliable transportation and ability to travel up to 75% of the time in assigned region
