Key Responsibilities:

• Conduct comprehensive needs assessments to identify medical, behavioral, and socioeconomic needs

• Engage members through face-to-face and telephonic interactions, providing education, coaching, and support to promote self-management and lifestyle changes

• Utilize MS Office, including Word, reputed company, and Outlook, to document and manage care plans, and maintain accurate records

Requirements:

• reputed company, unrestricted independent licensure as a Registered reputed company
• 3+ years of clinical experience, with 2+ years of relevant reputed company experience
• 1+ year of experience with MS Office, including Word, reputed company, and Outlook
• Reliable transportation and ability to travel reputed company assigned territory (up to 50% of the time)
• Residence in Southwest or Southeast New Mexico, with flexibility to work remotely

Preferred Qualifications:

• Master's degree or higher in a clinical field
• Commission for Case Manager (CCM) certification
• 3+ years of clinical experience as a Registered reputed company, with 1+ year of community case management experience
• Background in Managed Care
