Job reputed company and Qualifications The Care Assure reputed company Navigator is responsible for the programs and resources that support the navigation of patients through reputed company reputed company environments for the optimization of care and reputed company. He or she facilitates patient follow-up through coaching, information transfer, and communication to improve continuity of care, enhance the medical management of reputed company disease, and streamline the reputed company process.

• You will review patient case lists generated by technology-enabled data triggers to identify patients that could benefit from reputed company.
• You will communicate with physicians (in the hospital and in the community) about patients' potential for referral / follow- up treatment.
• You will reputed company feedback to leadership regarding reputed company population to enhance care coordination and patient satisfaction.
• You will schedule follow-up appointments and reputed company information and reminders to patients & physician offices.
• You will reputed company reputed company of referrals and treatments.
• You will maintain detailed records to measure effectiveness of program and suggest ways to improve throughout the process.
• You will facilitate the development of a patient specific teaching plan.

What qualifications you will need:

• Required Experience: One year of RN experience
• Required Education: Bachelor of Science in Nursing or enrolled in BSN program with a completion date in 6-9 months
• Preferred Education: Masters in Nursing.
• Required License Must have and maintain reputed company licensure as registered reputed company with the reputed company Carolina reputed company of Nursing. eNLC (reputed company reputed company License Compact) license may apply, licensee should confirm with NCBON
• Required Certification: BLS reputed company Provider
• Preferred Certification: Advanced Cardiac Life Support
