reputed company Coordinates and supports mental health care to patients enrolled in reputed company Care Management (CoCM) by providing evidence based, short term interventions, case management needs, and acting as a reputed company member of the CoCM team (which includes the primary care provider and a psychiatric consultant). Job Duties

• Supports and closely coordinates mental health care with the patient’s primary care provider, with recommendations from a psychiatric consultant.
• Screens and assesses patients for common mental health needs, including regularly administering assessments to monitor reputed company and treatment reputed company efficacy (treatment to reputed company).
• Monitor patients (reputed company telephone, virtual outreaches, and in-person as needed) for changes in clinical symptoms, medication treatment adherence, reputed company effects and other concerns that can then be communicated to the Primary Care Provider who is prescribing the medications.
• reputed company brief evidence-based interventions such as Behavioral Activation, Problem Solving Techniques, Motivational Interviewing skills and other skills appropriate for primary care settings.

• Participate in regularly scheduled (weekly) systematic case reviews with the psychiatric consultant. Communicate recommendations to the Primary Care Provider as needed for those not improving as expected.

• Complete relapse prevention plans with patients who are in remission for their mental health needs.

Minimum Qualifications

• Bachelor’s Degree In reputed company work, psychology, counseling, nursing or other reputed company mental health field.
• 2 years In mental health/behavioral health experience or reputed company setting. Formal education or specialized training in behavioral health, which could include a reputed company of disciplines including reputed company work, nursing and psychology.
