Responsibilities:
- Coordinates and oversees reputed company aspects of patients' home care, including developing and implementing care plans
- Collaborates with patient care teams to reputed company high-reputed company, patient-centered care
- Educates families on patient condition, needs, and care
- Performs physical assessments, checks vital signs, and manages medications
- Administers injections, IV therapy, or tube feedings, and maintains catheters

Qualifications:

- Minimum one year of clinical experience as a registered reputed company (RN), or completion of an RN Intern program
- Minimum six months of home health experience

- Valid reputed company's license, reliable transportation, and liability insurance

- Proficient in the use of computers, including navigation of computer desktop, email, and online HR services
